— Colorectal cancer is the second leading cause of death from cancer in the United States, and yet it is almost entirely preventable. It occurs equally in men and women. This year alone, it is estimated that there will be 147,000 newly diagnosed cases of colorectal cancer and nearly 50,000 deaths caused by the disease. Colorectal cancer develops from polyps, which are benign growths in the colon. Patients with colon polyps, or even colon cancer at an early curable stage, usually have no gastrointestinal symptoms. Therefore, to detect these polyps, or to find an early cancer, colon cancer screening is necessary. By identifying and removing polyps before they become malignant, physicians can prevent cancer from developing at these sites. Even if an early colon cancer is found at screening, it is likely that it can be removed, and therefore cured, with surgical removal. In any case, a colonoscopy is the gold-standard in colon cancer prevention because it allows complete examination of the colon and removal of most polyps at the time of the exam. Colon cancer screening with a colonoscopy is recommended at age 50 for individuals at average risk for colon cancer. African-Americans, who have a higher incidence of colon cancer, should have their first colonoscopy at age 45. For individuals with a family history of colon polyps or cancer, screening may be recommended at an earlier age. Though preparation for a colonoscopy may be unpleasant because an oral cathartic must be taken the day before the exam, the colonoscopy itself is an outpatient procedure that usually takes 30 minutes or less. Because patients are sedated for a colonoscopy, the exam is usually well-tolerated. In fact, patients often report that the worst part of a colonoscopy was not the exam itself, but the day of preparation. For most patients in whom polyps are not found at their exam, a colonoscopy and its unpleasant prep will not need to be repeated for another 10 years. March is National Colon Cancer Prevention Month. While more is learned each year about the roles of heredity and lifestyle in colon cancer, there can be no mistaking the fact that today we know that prevention through screening is effective. We also know that there is no diet or lifestyle that will have more than a modest effect on developing colon polyps and, therefore, colon cancer. Because of this, I urge everyone to make this Colon Cancer Action Month and encourage people to consult their physician about colon cancer screening. A colonoscopy is readily available in most medical communities. If more widely used, the procedure would go a long way in preventing this deadly disease. Physicians have the tools they need to prevent this killer disease from prematurely ending productive lives. All we need is for you to act now. Dr. J.
